Supervised Driving Program

Learner Driver’s Name:

This form is aimed at those learner drivers who are under the age of eighteen years of age and who wish to partake in the Valley Driving School’s Supervised Driving Program, having met the basic criteria.

To the Learner Driver:

As a learner driver participating in the Valley Driving School’s Supervised Driving Program, I agree to follow promptly and without argument all the instructions/requests given to me by the Supervisor. I also accept that the Supervisor has the final say on all matters related to the driving experience and is free to exercise any such judgement at their discretion. This includes me relinquishing driving for that particular session with the Supervisor assuming all driving duties. I also consent to sharing the vehicle with the Supervisor for the express purpose of supervising my driving in my quest to accumulate the required 120 hours of driving experience as determined by Vic Roads. I also consent, where appropriate, for the Supervisor to dispense first-aid to me in case of injury and take whatever actions, where practicable, in a manner that any reasonable person would ordinarily expect.

To the Parent/Guardian:

As the parent/guardian of                                                       , I give consent for her/his participation in the Valley Driving School’s Supervised Driving Program for the express purpose of providing supervision for my daughter’s/son’s driving experience in their quest to accumulate the necessary 120 hours driving time. I also consent to passing full authority to the Supervisor to make whatever judgement calls they deem necessary with respect to my daughter’s/son’s driving capacity in the name of safety and due care. In the case of an emergency, I authorise the Supervisor to execute whatever course of action they can sensibly perform, including first-aid, and where practicable (given the circumstances), in a manner that would ordinarily be expected of any reasonable person.
